With your needs in mind, we're excited to announce our partnership with Ceras Health, a leader in digital healthcare.  As part of this partnership, we're introducing a CGM365service to all our patients using Continuous Glucose Monitoring (CGM) devices. This service offers round-the-clock blood glucose monitoring, bringing peace of mind and ensuring your child receives the best possible care.

How much will this cost?

This service is free for Medicaid and commercial insurance.

How to Enroll in CGM365

Patients/Caregivers can self-enroll in the Ceras CGM365 program through the "Register Now" button below to get started. Once registration is complete, you will be given a link to the  CerasEmpower App to pair your CGM device and get started with CGM365!

Your Questions Answered:

  • Downloading the App:

    Q:  How does an iPhone user locate and download the Empower app? 

    For iPhone users, the App Store icon is a blue square with a white ‘A’ in the center. 

     

    Q: How does an Android user locate and download the Empower app? 

    For Android users, the Google Play Store icon is a multi-colored triangle icon. 

    Q: What does the icon for the Empower app look like? 

    The Ceras Empower app is a gray background with a white bumblebee. 

     

    Q: What information may be needed to download the Empower app? 

    Users may need to know their apple ID/password or their google ID/password. 

  • Logging in/registration:

    Please Register for CGM365 before downloading and logging into the CerasEmpower App.

     

    Q:  The number entered to login to the app keeps saying “user not found”, what should I do? 

    Contact the customer support team at 877-723-7277.   

     

    Q:  The OTP code won’t send to my phone, what should I do? 

    Wait two minutes, then try to resend the code by tapping ‘resend’.  If this does not work, you can log in via email and password by tapping ‘alternatives to connect’. 

     

    Q:  There is a video that keeps popping up on the screen with a woman talking, how do I move past this without watching it? 

    At the bottom of each screen is a ‘skip’ button.  Tap skip to move on to the next section of the app. 

     

    Q: I am already enrolled in Ceras services, why is the app asking me to complete enrollment? 

    This enrollment is for the app, you can skip this step if you wish.
